All co-payments with "in network" insurance are payable in full at time of service.

We are always in the process of contracting with other Insurance Companies. If you don't see your insurance company listed, please call and ask.

If your insurance company in not "in network" with us, full payment is due at time of service. We will provide you with the necessary paperwork so that you may file a claim with your insurance yourself.

Uninsured and "out of network" insurance patients receive a 25% discount for cash payments.

Please note: We are NOT MEDICAID PROVIDERS and therefore will not be able to provide services to Medicaid cardholder members (including ALL branches of Medicaid...Staywell/Wellcare, Medipass, Amerigroup/Ameripass, and Florida Kid Care.
